OS3/NovitreX®: perfection in 23G ppV surgery

3000 efficient cuts per minute with the Oertli 23G Twinac stripper, availability of flow controlled peristaltic or vacuum controlled venturi aspiration at any time during surgery, best contrast vision with two independent metal halide illumination sources and the Oertli PMS pars plana Microincision system make 23G vitrectomy easy and stress free. Now Oertli has made a further step forwards: Autoseal® PMS eliminates the need for plugs, keeps the pilot tubes almost perfectly tight, maintains the IOP constant throughout surgery and allows free rotation of the infusion terminal. Read more in Application Note 2/2008. The Autoseal® PMS components are available as single use sterile items (VV123110) or in a complete vitrectomy machine pack with cassette, cutter, illuminator, air line, stop cock (VV630013A) in boxes of 10.

OS3 opens the path to 1.6 mm coaxial phaco

The OS3 machine now opens access to the most advanced MICS technology. The CO-MICS 2 system offers efficiency and phaco times as short as with conventional 2.8 mm phaco, but with improved fluidics performance: nearly perfect chamber stability and elimination of post occlusion breaks, excellent holding force and followability. The physical background of this convincing new concept is described in Oertli Application Note 1/2008. No new surgical technique is required to do CO-MICS 2 coaxial phaco with 1.6 mm incisions (1.4mm to 1.8mm range possible). The OS3 needs no modification or special software. Just change the blade and set aspiration vacuum 150 mmHg higher than with your preferred current technique. The CO-MICS 2 components are available as single use sterile items (VV800833) or in a complete machine pack with cassette (VV630111C)in boxes of 10.

OS3/NovitreX® now with “Good Light” dual light source

The vitreoretinal extension unit NovitreX now comprises two parallel metal halide light sources. The term “Good Light” is justified. It is a good light in every aspect: bright illumination, excellent contrast viewing and visibility of fine structures but optimum prevention of toxicity thanks to carefully selected spectrum and energy levels. Two independent light sources bring best security in case of lamp failure and allow two light instruments to be used in parallel. The NovitreX module VC830201 also includes air and silicon infusion/aspiration.

NovitreX3000® high speed vitrectomy increases efficiency

The OS3 now drives the Oertli Twinac strippers with cutting rates up to 3000 cuts per minute. Investigators were surprised about the true efficiency and usefulness of this feature. The Twinac/OS3 concept uses compressed air for both, the cutting and the aspiration cycle of the guillotine cutter thereby providing perfect control of both processes. The result is efficient but tension free vitreous removal with reduction of overall vitrectomy times. In particular 23G surgeries now show vitreous removal times in par with 20G surgery.

Parsplana Micro incision System PMS opens path to the future

The advantages of the new 23G and 25G technology over the conventional 20G technology are clear. With an advanced concept, Oertli has moved beyond the initial difficulties associated with miniaturization. The future of vitrectomy is now in your hands!

The Oertli PMS follows the tradition of the pilot tube concept developed by Professor Klöti in 1973. In contrast to trocar systems, the transconjunctival incision is not made via radial puncturing with a polished cannula and attached tube. The Oertli PMS involves the use of a precisely tuned MVR blade. This blade permits perfect incisions and rapid healing. The incision is executed as a tunnel incision with a flat angle. Self-sealing is consistently excellent.

The exact position of the incision is determined using a template, with a repositioning of the conjunctiva in relation to the sclera being achieved simultaneously. The template is removed only after the pilot tubes have been placed.

The single-piece pilot tubes in the Oertli PMS system are precision manufactured of solid titanium. This is why they slide ideally through the incision and remain in place. In contrast to synthetic tubes, they cannot be damaged during placement and when changing instruments. They also offer optimal properties for instrument insertion.

A step-by-step illustrated description of PMS use is contained in the Application Note 6, “Microincision in Vitrectomy”.

20G Phaco pack for 2.2mm incisions

OS3 phaco packs are now available with the 20G micro tip system. The micro tip allows 2.2mm incisions and provides excellent followability. The packs include the venture/peristaltic Twinvac cassette with giving set plus the 20G Excellerator SU tip (30°) with infusion sleeve and test chamber.
